CONSENT/CONFIRMATION OF DIAGNOSIS FORM.  This form allows cardiologists to release pedigree and other information to help the breed deal with cardiomyopathy. To be signed by the owner and cardiologist, and a copy sent to Dr Bruce Cattanach, the geneticist collating the information.
